PROOF OF RESIDENCY 

• A current utility bill (within 30 days) showing the name and address of the student’s and

  parent’s home.

• A home purchase contract including a specific closing date with a copy of the deed to be provided within 90 days.  

• Mortgage or Rental Contract

• An approved School Choice application

• Documentation of an approved legal transfer between school districts.

• Families living in a situation where these documents are in another person’s name will need to complete the Head of Address Verification Form (Click Here)

• If you answer Yes to the question in the registration “Is your child homeless?”, then please complete the McKinney-Vento form McKinny-Vento Form (Click Here)

• Refer to the Prairie Grove Student Handbook if you have any questions regarding this policy.   (Student Handbook)

BIRTH CERTIFICATE 

• Refer to the Prairie Grove Student Handbook to see what other documents are accepted if

  a Birth Certificate is not available (Student Handbook)

SHOT RECORDS

• Refer to the Prairie Grove Student Handbook if you have any questions regarding your child’s immunizations, (Student Handbook)

SOCIAL SECURITY CARD

• Refer to the Prairie Grove Student Handbook if you have any questions regarding providing your child’s social security card (Student Handbook)

HEALTH PHYSICAL

• This is required for Preschool and Kindergarten students only. 

• Refer to the Prairie Grove Student Handbook if you have any questions regarding this 

IF YOU ARE NOT ABLE TO UPLOAD THESE DOCUMENTS, YOU WILL NEED TO PROVIDE THESE TO THE REGISTRAR AT THE ELEMENTARY SCHOOL.

REGISTRATION IS NOT COMPLETE UNTIL ALL DOCUMENTS HAVE BEEN PROVIDED AND ENROLLMENT HAS BEEN APPROVED.

How do I register my student without internet services at home?

• The District will be providing an onsite online registration for those without internet services at home. If you are needing this service, please contact the Registrar at registrar@pgtigers.org or Click Here to schedule a time to register.

• New Student Information on Class Options

• Once Registered, Click to Schedule a time to set up classes with the Counselor

After you submit your student's registration online, the record will be processed and reviewed by the District Registrar. If there are any problems with your registration, you will be contacted by the District Registrar.

REGISTRATION IS NOT COMPLETE UNTIL ALL DOCUMENTS HAVE BEEN PROVIDED AND YOUR POWER SCHOOL ONLINE ENROLLMENT HAS BEEN APPROVED BY THE DISTRICT REGISTRAR.

What is the primary way to register my kindergarten or new student?

You can register your kindergarten or new student online beginning Mid-July. Kindergarten students must be 5 by August 1, 2025.

Yes, enrollment documents are required to finalize registration.
